Police had been questioning the two over the weekend after raids on their homes.
Dr Mullany and her husband, both 31 and from Pontardawe in the Swansea Valley, were shot on 27 July.
The two accused appeared before magistrates in St John's, the capital of Antigua on Monday.
Dr Mullany died instantly and her husband died after he was airlifted back to south Wales.
The couple were staying at the Cocos Hotel and Resort on the south west of the island when they were attacked.
They had married just two weeks before in a church near their south Wales home.
